Initial Assessment and Testing
When Clients of the Prakash Ellenhorn PACT Team first enter the program, they engage in a month-long assessment process. The assessment includes a full battery of psychological testing, a neurological evaluation if necessary, and medical, vocation, spiritual, substance abuse, family and social/developmental assessments. During this process, they typically receive approximately fifteen to twenty hours of service a week.
Cost of month-long assessment: $17,500
PACT Services
Once a client has finished his or her first month, the team meets and develops a treatment plan. The plan, which provides a roadmap for all the contacts with the client’s multidisciplinary team, sets a predicted amount of care management hours that the client will need in the months to come. The intensity of care, however, shifts over time, as the team contours contacts to the client’s needs. On average, most clients receive fifteen hours of care management a week.
The monthly costs for the PACT program are as follows:
- 30 hours a week of care management for one month: $19,500
- 20 hours a week of care management for one month: $14,500
- 15 hours a week of care management for one month: $11,300
